How many hours a day should the harness be worn?
Usually 23 to 24 hours a day during the initial phase, reducing gradually during weaning. Your surgeon sets the exact hours.
Can I take the harness off for a bath?
Only if your clinician says so. Many protocols require continuous wear at first; sponge bathing is used instead.
Can I adjust the straps myself?
No. Strap positions are prescribed and checked on ultrasound. Adjust only when specifically instructed.
Does the harness hurt my baby?
A correctly fitted harness is not painful. Fussing for the first day or two is normal; persistent distress is not.
My baby cries every time I put it on. Is that normal?
In the first 48 to 72 hours, yes. After that, reapply the harness and call your clinic if crying continues.
Can my baby be breastfed in the harness?
Yes. A rugby-ball or laid-back hold usually works best because the legs are bent and spread.
Will the harness delay walking?
Motor milestones may be a few weeks behind during treatment and then catch up. Walking almost always arrives within the normal range.
Can we still do tummy time?
Yes, with a rolled towel under the chest. Tummy time remains important.
What clothes fit over a Pavlik harness?
Dress one to two sizes up. Wide-legged rompers, poppered bodysuits, skirts and leg warmers all work; narrow trousers do not.
Should the nappy go over or under the straps?
Always under the straps. Use a nappy one size up.
How do I change a nappy without moving the hips?
Slide a hand under the bottom and lift the pelvis rather than lifting the legs by the ankles.
Can I use a baby carrier?
Yes, if it supports thigh to knee in the wide M position and does not press the legs together.
Will the harness fit in a car seat?
Most rear-facing infant seats work. Test before travelling; never loosen either harness to make it fit.
Can we fly with a Pavlik harness?
Yes. Carry your clinic letter and a diagram of the strap settings, and keep a spare harness in hand luggage.
How do I wash the harness?
30 °C delicate cycle in a mesh bag with fragrance-free detergent, air-dried flat. Never tumble dry.
Do I need a second harness?
It is not required, but a spare makes washing and travel far easier.
What if the skin gets red?
Marks that fade within 20 minutes are normal. Persistent redness or broken skin needs same-day clinical review.
Can I put cream or powder under the straps?
No. They trap moisture and clump. Keep skin dry and use a thin cotton barrier layer instead.
My baby has stopped kicking one leg. What do I do?
Contact your clinic urgently. This can indicate femoral nerve compression, which is usually reversible when treated promptly.
Can the harness cause avascular necrosis?
Excessive abduction can compromise blood supply to the femoral head. This is why abduction is set by a clinician and never increased at home.
How soon will I know it is working?
Ultrasound at one to two weeks confirms reduction; socket improvement is usually visible by six weeks.
What is the success rate?
Roughly 85 to 95 percent overall for reducible hips treated early, and higher for mild dysplasia.
What happens if it does not work?
The usual next step is examination under anaesthetic with closed reduction and a hip spica cast.
How long is the whole treatment?
Typically 6 to 12 weeks of full-time wear plus several weeks of weaning, then imaging follow-up until walking age.
Will hip dysplasia come back?
A minority of children develop residual dysplasia, which is why follow-up X-rays continue to walking age.
Is a harness the same as a spica cast?
No. A harness is soft, adjustable and removable; a spica cast is rigid plaster or fibreglass applied under anaesthetic.
What size harness do I need?
Size by chest circumference: Small 33–38 cm, Medium 38–44 cm, Large 44–50 cm. When in doubt, size up.
