Harness treatment is a sequence of short appointments rather than one big event. Knowing what happens at each takes a lot of the anxiety out of the calendar.
Schedules vary between units, but the shape is remarkably consistent.
Visit 1 — fitting (week 0)
Measurement, harness fitting, and setting of flexion and abduction angles, followed by an ultrasound to confirm the femoral head is seated. You will be shown how to change nappies and dress the baby without removing the harness. Ask for the strap positions to be marked and photograph them before you leave.
Visit 2 — confirmation (1-2 weeks)
The most important early appointment. The clinician repeats the ultrasound with the harness on to confirm reduction, checks skin, and lets the straps out for growth. If the hip is not reduced at this visit, expect a change of strap settings and a short-interval re-scan.
Visits 3-5 — progress (weeks 3-8)
- Skin check under every strap.
- Femoral nerve check: is the baby actively kicking both legs?
- Strap adjustment for growth — usually 1-2 cm at a time.
- Repeat ultrasound at roughly three to four week intervals, tracking the alpha angle.
- Discussion of weaning once the angle is normalising.
Weaning visits
When imaging shows a mature socket, wear reduces in steps — often full time to 16-18 hours, then nights and naps only, then nights only. Each step is typically two to three weeks with a check in between. Some units re-scan after each reduction in hours.
Discharge and long-term checks
A final ultrasound or X-ray confirms the hip is normal. Most surgeons then request an X-ray around walking age, and some add one in mid-childhood, because a small proportion of hips develop residual dysplasia even after successful early treatment. Keep these appointments even when your child is running around perfectly.
